15-count indictment announced against Forsyth dentistry operator in Medicaid fraud case

FORSYTH COUNTY, Ga. — Georgia Attorney General Chris Carr on Thursday announced a 15-count Medicaid fraud indictment against a Forsyth County dentist.

A release from AG Carr's office said 48-year-old Farid Moghaddam, who operates North Cumming Dentistry, "is alleged to have submitted fraudulent claims for services he did not provide and to have then retained payments for those claims."

The indictment lays out allegations that Moghaddam improperly collected nearly $40,000 through claims submitted for four different Medicaid patients from 2016-2022, in amounts between roughly $6,700-$14,000.

His full charges include four counts of felony Medicaid fraud and 11 counts of felony false writings, Carr's release said.

“We’re committed to protecting taxpayer dollars by putting a stop to Medicaid fraud in our state,” Carr said in a statement. “Ensuring the integrity of providers and services is a key part of our efforts. We won’t tolerate those who violate the public’s trust by abusing a system meant to care for our most vulnerable Georgians.”
